The Sheffield Parents’ Survey

We want you to share your experiences and views as a parent / carer by completing The Sheffield Parents’ Survey which can be found here:

http://www.snapsurveys.com/swh/surveylogin.asp?k=134460543905

Your responses will help council and partner organisations (including schools) to plan, deliver and commission appropriate services in a more effective and efficient way.

The survey, which only takes around 10 minutes to do, is aimed at all parents and carers in Sheffield. It asks important questions about the area where you live, parenting & family life, health, childcare, safety, crime & antisocial behaviour and schools.

We do not ask for your name or address but there are some additional questions in the ‘About You’ section at the end which will help us to ensure that we include a cross-section of parents and carers across the city.

As well as helping to shape services that affect you and your family, you could also win something from our wide range of prizes – all sourced from Sheffield businesses, shops and enterprises.

If you would like to be entered into our prize draw, there is an online form at the end of the survey where you can enter your name and contact details.

The survey runs until October half-term. Its results will be published on

www.sheffield.gov.uk/sps from December 2012.

***If you require a paper copy of the survey, you can call 0114 205 3902 or email
